Thank you for your interest in holding an event with CBS.

Please complete this form a least 3 weeks ahead of your desired event date so we can help make your event a success.

Be sure to answer all questions in as much details as possible, where applicable. Before completing this Program Proposal Form, please look at the CBS calendar at cbsnapa.org/calendar to see if your preferred date/time looks available. (Note that behind-the-scenes activities may impact actual availability.)

If you have a change to a previously submitted event, please complete the form below or send an email to office@cbsnapa.org with the words, "CHANGE for 'Program Name' " in the subject line.
This will be your confirmation email address
If reoccurring please include how often.
Please indicate if this is per household or per person.
If left without an upload the CBS office will work with you to create a graphic.
To ensure resources are used responsibly we ask that you confirm that a minimum of 6 individuals will be participating. Please list their names in the space above.
